Are nitazines new?
No.
Developed in the 1950s in Switzerland, Nitazines are extremely potent pain killers.
Essentially they open up opioid receptors allowing for the transmission of naturally occurring chemicals that block pain signals.
The side effects of Nitazines were considered too great for the drugs to be marketed as pharmaceutical drugs so were never used in human or animal medicine.
Early in 2019 the illegal drugs market began to see Nitazines being used in North America and then Europe.
Very soon after, fatal overdoses were being seen.
The major problem with Nitazines is that they have a low therapeutic index.
This means that there is a very, very small margin between a therapeutic (safe) dose and an overdose.
Isotonitazene was the first Nitazine that was discovered as being used in 2019.
Nitazines are now at the centre of a full blown crisis in the UK and eastern Europe. Over 400 people in the UK have already died from using them.

How big is the problem?
Evidence is emerging that Nitazines are being mixed with both heroin and cocaine.
This evidence has been gained, partly, through the National Crime Agency operation Ropey (2021) where forensic tests were carried out on seized drugs.
Over a quarter of seized cocaine had traces of Nitazines and over half of the seized heroin did as well.
In scotland, Nitazines have been found in fake opioid tablets.
Nitazines can be inhaled, smoked, or swallowed as tablets. It can also be injected.

What effect do they have?
Like all opioids, Nitazines produce a feeling of warmth, deep relaxation and a great detachment from reality.
They also lessen anxiety.
As they are opioids they will block pain signals.
The effects of Nitazines are like other opiates, usually giving feelings of warmth, relaxation and detachment with a lessening of anxiety but also dizziness, nausea and vomiting.
They will also depress the respiratory system which can lead to fatality.

Why are they addictive?
Nitazines are opioids so are highly addictive.
Because Nitazines are so much more powerful than opioids it is very easy to overdose.
When you take opioids, they affect receptors on the brain by opening up opioid receptors which increase the flow of naturally occurring chemicals which block pain signals.
After a time the brain becomes used to them and will change the way it works.
A state of tolerance occurs.
This means that you will need more and more to have the same effect as the small amount you started with.
Nitazines have a very narrow therapeutic index.

Cravings and withdrawal
Just as with Heroin withdrawal, if you stop taking Nitazines you are going to go into a state of withdrawal. Cravings are an important factor to consider.
Your brain simply can not adjust to the change. It takes time.
While it does so you will experience, within hours, significant physical and psychological symptoms:

Going cold turkey
This is the process of not providing medical intervention.
It is known as cold turkey as the skin will resemble that of a goose or turkey.
Going "cold turkey" is extremely dangerous.
It can be fatal.
Even if you did make it through, the cravings will be so intense that you are very likely to succumb.
When people try to detox themselves at home and relapse, due to cravings, an overdose is almost inevitable.

What about home treatment for Nitazine addiction?
No.
This would simply not be clinically viable, safe or even something anyone should consider.
The only safe way to recover from Nitazine addiction is with a medically supervised detox using a substitute medication under close medical supervision.
While this could be done in the home environment it would be very cost prohibitive and would always cost far in excess of even the most high end rehab centre.
Therapy
All centers who provide treatment for nitazine and opioid addiction provide a fully immersive therapy programme, either along a 12 step approach or a SMART based recovery model. These run seven days a week and , those in treatment, are obliged to attend all sessions if physically well enough to do so. For new admissions, it can be 24-48 hours , on occasion, before someone is able to take part.
